Job Description

Role Overview:The Home Health RN provides skilled nursing services to patients in their homes, supporting recovery and long-term health management. This position requires independent clinical judgment, strong organizational skills, and a focus on delivering patient-centered care. Key Responsibilities: * Assess patient health status during home visits and identify care needs * Develop and carry out individualized care plans in accordance with physician orders * Provide nursing care such as medication administration, wound care, and monitoring of chronic conditions * Track patient progress and adjust care as necessary * Maintain clear, accurate, and timely clinical documentation * Educate patients and caregivers on treatment plans, medications, and home care techniques * Communicate effectively with physicians and other healthcare providers * Promote safety and infection control within the home environment Qualifications: * Active Registered Nurse license * Graduate of an accredited nursing program * Current CPR/BLS certification * Previous clinical experience required; home health experience preferred * Strong critical thinking and assessment skills * Ability to work independently and manage a caseload * Reliable transportation for travel between patient homes Work Setting: * Patient care delivered in private homes * Local travel required on a daily basis * Flexible scheduling based on patient assignments Physical Requirements: * Ability to assist patients with mobility and daily care * Frequent standing, walking, bending, and lifting * Ability to safely handle medical equipment Benefits (optional): * Flexible work schedule * Mileage reimbursement * Health insurance options * Paid time off * Opportunities for continuing education
