Assignment Snapshot Discipline: Occupational Therapist / OT Setting: Skilled Nursing / Post-Acute Rehab Facility / Area: Portsmouth Health and Rehab Location: Portsmouth, VA Assignment Length: 13-week assignment Start Date: ASAP Schedule: Mon-Fri Guaranteed Hours: 35 guaranteed / 40 scheduled Estimated Weekly Gross: $2,366 Estimated Weekly Net: $2,090 What You’ll Do Provide skilled Occupational Therapy services in accordance with clinical standards, physician orders, plan of care, and facility expectations. Evaluate, treat, document, and help patients improve function, independence, safety, ADLs, mobility, and quality of life. Develop and update individualized treatment plans based on patient needs, goals, and progress. Collaborate with rehab leadership, nursing, facility teams, families, and care partners. Support discharge planning, patient education, caregiver education, and functional outcomes. Step into the assignment with professionalism, flexibility, and a patient-centered approach. What We’re Looking For Active or eligible Virginia Occupational Therapist / OT license. Prior skilled nursing, post-acute rehab, LTC, or geriatric experience preferred, NG accepted. Travel experience preferred, but not required for the right clinician. Net Health experience is a plus, but not required. Strong documentation, communication, and team collaboration skills. Ability to start quickly and complete required onboarding before assignment start. Why Travel with Continuum?
